What documents should I bring?

All pertinent medical records will be transferred to us by your doctor. Please bring the following documents on the day of your admission: Medicare, supplemental insurance, private insurance, VA, and Medicare Part D cards, and any Long Term Care insurance information. We will also need a copy of your medical and financial Power of Attorney, …
